REGISTRATION FORMS
Registration Instructions for Ward Young Men and Young Women Leaders
1. Be familiar with the website http://www.santabarbaratrek.com.
2. Print out or photocopy
a. Registration Form and Medical & Release Form
b. Have one set of forms per youth registrant and one form per adult leader registrant (this is a different registration form - see
below for the downloadable version).
c. The Standards of Dress and Conduct Form accompanies the registration form given to the registrant but is intended to be kept and followed.
3. Have each registrant fill in the forms and sign where appropriate.
a. Signature: It is MANDATORY to have each registration form signed.
b. Ward leaders must review the completed form for completeness and accuracy.
c. Incomplete and/or inaccurate forms will be sent back to leaders for revision(s).
4. Registration Form requirements
a. Ages: All youth 14 through 18 are invited.
New Church guidelines allow youth turning 14 anytime in 2023. If 18 must be graduating high school in 2023.
b. Non-members and out-of-area family members are invited to attend if they have been approved by the sponsoring Ward Bishop, they meet the other attendance requirements, and the Registration Form is completed and accurate (including parental permission)
i. Out-of-area family members may not be assigned to the same handcart family as their sponsoring Ward or Youth Member
ii. Non-members may need a special hand of fellowship, so every effort will be made to have one member known to them in their handcart family
c. Deadline:
i. Wards – Send completed forms to your respective Stake YM/YW leaders by April 30, 2023
ii. Stakes – Scan all forms into two files: one file for all the youth of your Stake and one file for all the adults attending from your Stake (including mas and pas). Email those files to the email address below no later than May 8, 2023. Prior to emailing those files, please complete the 2 excel spreadsheets that will be emailed to a Stake Representative for your Stake (one spreadsheet for youth, one for adults).

e. Late: If a late registration is unavoidably necessary for a youth or adult leader, sending an electronic version followed by the paper version (required) is advised

5. Registration Sign-in is at Live Oak Campground
a. Signage and directions will made available to Stake leaders
b. Permanent and temporary Trek 2023 parking will be available on-site
c. Non-Trek adult leaders and Parents are invited to leave after delivery or drop-off to lessen confusion and congestion on-site
d. ALL Trek-participating adult leaders are required to both pre-register, and to sign in the day of Trek, this includes full-time leaders and part-time leaders - those remaining overnight as well as those driving to the site later
6. Special arrangements
a. If you feel you need to make a special arrangement, not all of which can be honored, please do so as far in advance as possible.
b. Last minute accommodations, many of which require levels of approval and decisions, unnecessarily burden directors and leaders with last-minute decisions and workarounds (possibly delaying Trek departure) – PLEASE avoid at all possible costs.
In addition, below is a downloadable PARENT CHECKLIST sheet. This is a tool to assist parents in helping to prepare their youth for this Trek experience. It would be helpful to print and distribute to parents or direct them to this website.
